How to Enroll in Christ's Greenfield

                                                                  Enrollment Checklist


Thank you for your interest in Christ’s Greenfield Lutheran School. All of the staff and students
look forward to the best year ever. God be praised!

Please consult the following checklist to be sure that all steps of your application have been
completed.

When you register:

  •  Registration Fee - $299
  • Application for Enrollment Form
  • Schedule your Family Interview with the Principal – required for acceptance


Required prior to Family Interview Appointment:

  • Report Card * – most recent
  • Standardized Test Results* (most recent school year)
  • Immunization Records (copy required for all students)
  • Birth Certificate (copy required for all students)
  • School Year Enrollment Contract (2 pages)
  • Tuition Payment Worksheet
  • Private School Affidavit –must be notarized
  • Custody/Court Documents (required for students with custodian concerns)


All of the above should be turned in to the School Office at least 24 hours prior to the Family Interview appointment. If you have questions, comments or concerns, please contact the office.

Due after acceptance, prior to first day of school:

  • White Emergency Card
  • 4x6 Emergency Information Cards (2)
  • Authorization Pick Up List
  • Extended Care Registration Form
  • Official Withdrawal Form from previous school * (copy required for all students)
  • Signed consent form for previous school to release records to CGLS
  • First Month’s Tuition process through FACTS management



                                  * Not a required item for Kindergarten students
